how is a Breast Reduction performed?

All cases are assessed individually, but in general there is one preferred breast reduction approach. The technique used to reduce the size of the breasts is very similar to that of a Full Mastopexy. This includes an incision around the areola, vertically down to the crease then along the breast crease horizontally, making an anchor shape. (Although for most patients, the incision under the breast crease is not seen). The areola is kept attached to maintain blood flow and simply shifted to a higher position on the breasts. This is the preferred method to be used if there is an extreme degree of sagging on heavy breasts.



Asymmetry affects everyone! Most women have one breast that’s bigger than the other just like they might have a leg or arm that’s slightly longer than the other. (And by the way, when you have your next pedicure, look and see if your toes exactly match!) The point is that it’s rare for the human body to be symmetrical and most patients with large breasts have moderate asymmetry. Dr. Aiello’s approach to breast reduction maximizes the possibility that your outcome is as symmetrical as possible, even in instances of extreme asymmetry.

It was recommended that I have implants with my reduction. I don’t want bigger breasts!

In some select cases we do breast augmentation at the time of the breast reduction. This may seem paradoxical in that we are simultaneously making you smaller and also making you larger. The reason is that, although some women have large, heavy pendulous breasts, most of the breast tissue is below the level of the nipple and even with the reduction and lifting, this will persists. Therefore, despite the elevated position on the chest after the breast reduction, there is very little upper breast volume which gives women the fullness they want with a smaller breast. This procedure is not done by many plastic surgeons, but in certain cases Dr. Aiello recommends it and at Ocean Plastic Surgery Center we have been doing breast reductions with augmentations for years with great success!

how common are complications with Breast Reduction surgery?

Although complications are rare with this procedure, there are a few that are of concern, and it is worth noting, smoking can seriously contribute to them. In patients with extreme cases of very large, pendulous breasts, there is a risk of nipple necrosis (when part or all of nipple does not survive). This is due to blood circulation problems and smoking can seriously compromise the healing process. Dr. Aiello will discuss all your options at your consultation to help you make the best decision when it comes to your reduction. He continues to have great success even in these cases of maintaining blood circulation to the nipple, but it must be noted, there is an option a graft may be needed.

Another rare, but valid concern are hypertrophic and keloid scars. These usually manifests in individuals genetically predisposed. To minimize this, we watch you closely and can prophylactically put you on medications.


A breast reduction is a permanent procedure; however your results can change with weight gain or pregnancy. These both have the potential to add excess tissue to the breasts.
