Patients Right Under HIPPA

A. Right to a Notice of Privacy Practices
Patients must be given a copy of the Notice of Privacy Practices. We must post at each service location and on our web site, and provide it to patients upon request.

B. Right to Access Private Health Information
Patients have the right to get a copy of their Private Health Information (PHI) that are in our records. We can require that they make a request in writing and we can charge a reasonable fee for the cost to generate the copy.

C. Right to Request to Amendments to PHI
Patients have the right to ask us to amend or correct their PHI. We can ask them to request these changes in writing and we do not have to grant all requests.

D. Right to Request Alternative Means of Communicating PHI
Patients may request that we communicate their PHI by alternative means (fax) or to an alternative location. Requests can be made in writing.
